Agreement to Terms
By scheduling and attending therapy sessions with LeMaster Counseling Services, LLC, you agree to these terms of service. Please read them carefully.
Scope of Services
What We Provide:
Individual psychotherapy services for adults (18+)
Telehealth sessions via SimplePractice platform
Mental health treatment within the scope of my license and training
What We Do NOT Provide:
Emergency or crisis services
Medication management or prescriptions
Court-ordered evaluations or testimony
Services outside of Louisiana (until licensing expansion)
Client Responsibilities
For Successful Therapy:
Attend scheduled sessions on time
Participate actively and honestly in the therapeutic process
Complete any agreed-upon between-session activities
Communicate openly about concerns or questions
For Telehealth Sessions:
Ensure you're in a private, secure location
Use a reliable internet connection
Test your technology before sessions
Have a backup communication method available
Inform me if anyone else can hear our session
Scheduling and Attendance
Scheduling:
Sessions are scheduled through SimplePractice or by contacting me directly
Current availability: Monday - Friday Evenings; Saturday & Sunday Mornings
Session length: 50 minutes
Cancellations:
Minimum 48 hours notice required for cancellations
Less than 48 hours notice results in full session charge
Exception: If we can reschedule within the same week
No-shows are charged the full session rate
Late Arrivals:
Sessions begin and end at scheduled times
Late arrivals may result in shortened sessions at full cost
Payment Terms
Session Fees:
$125 per 50-minute session
Sliding scale available: $70-$125 based on need
Payment due at time of service
Insurance:
Currently not accepting insurance (Insurance Credentialing in Process)
Super bills provided for out-of-network reimbursement upon request
Payment Methods:
Processed through SimplePractice secure portal
Credit card, debit card, or bank transfer accepted
Limitations and Risks
Telehealth Limitations:
May not be suitable for all mental health conditions
Technology failures can disrupt sessions
Emergency response may be delayed compared to in-person services
Therapy Risks:
May initially increase emotional distress
No guarantee of specific outcomes
Requires active participation for effectiveness
Telehealth Specific Terms
Technology Requirements:
You are responsible for ensuring adequate technology and internet
Technical difficulties may require rescheduling at no charge
SimplePractice platform terms of use also apply
Location Restrictions:
Services provided only to clients physically located in Louisiana during sessions
You must inform me if traveling outside Louisiana
Emergency Procedures:
Telehealth is not appropriate for mental health emergencies
In crisis situations, contact 911 or go to nearest emergency room
I will provide local emergency resources as needed
Confidentiality and Records
Confidentiality:
All communications are confidential per HIPAA and Louisiana law
Exceptions only as required by law (safety concerns, court orders, etc.)

Records:
Clinical records maintained per Louisiana state requirements
You have right to access your records with written request
Professional Boundaries
Communication:
Business contact only via provided email and phone number
Non-emergency communications answered within 24-48 hours
Emergency situations require immediate professional help (911)
Dual Relationships:
Professional relationship maintained at all times
Social media connections not appropriate
Personal or business relationships outside therapy are avoided
Termination of Services
Either party may terminate services:
With appropriate notice and discussion
Client referrals provided when appropriate
Records maintained per legal requirements
Services may be terminated if:
Terms of service are violated
Client safety cannot be ensured via telehealth
Services are outside scope of practice
Legal and Liability
Jurisdiction:
Louisiana state laws govern these terms
Licensed only in Louisiana
Liability:
Services provided within standard of care
Not responsible for technology failures beyond our control
Client responsible for their physical environment during sessions
Changes to Terms
We reserve the right to update these terms. Changes will be posted on our website and discussed during sessions when material changes occur.
